The following details the terms and conditions of service, ensuring that you are able to make an informed choice regarding the right care for you and your child during this difficult time. Please feel free to contact me for any further information or details that will help put you at ease and aid your decision making.
All information within the website does not constitute individual professional advice. I can only provide accurate individualised and professional advice during a clinical consultation.
The Online Medical Questionnaire, Consent Form and Covid-19 Screening form are extremely important at this time and I will require personal information regarding both yourself and your baby. This may affect the advice I give and therefore the wellbeing of your child may be affected if you don’t complete these accurately and honestly. Please ensure that you complete the Medical Questionnaire, Consent Form and Covid-19 Screening forms BEFORE the scheduled appointment, otherwise it cannot go ahead as planned.
Please bring your child’s Red Health Record with you to the appointment also.
To ensure you are able to make the right informed care choice for you and your baby, I have added the updated Covid-19 Consent form to my website and also provided links to an online version to complete and sign, in the booking confirmation and reminder emails. This will be used as explicit proof of you giving full informed consent for me to assess and treat your baby during this unprecedented time. I will then discuss it during the face-to-face appointment also. Please contact me directly if you would like a hard copy of the consent form to sign and return to me in addition.
Currently during the Covid-19 pandemic, all payments for care will be required online at the point of booking. I use the online system Stripe and there will be no added fee for you, I pay the required fees to offer the service.
I hope that you will not be unhappy with my service, however if you do feel that there is a need to make a complaint it will be taken very seriously. Please contact me directly in writing to share your concerns and I will endeavour to resolve your complaint as quickly as possible.
