Booking Terms & Conditions
Thank you for visiting my website. The following details the terms and conditions of service, ensuring that you are able to make an informed choice regarding the right care for you and your child. Please feel free to contact me for any further information or details that will help put you at ease and aid your decision making.

Information giving
All information within the website does not constitute individual professional advice. I can only provide accurate individualised and professional advice during a clinical consultation.

The consultation process is very important and I will require personal information regarding both yourself and your baby as this may affect the advice I give and therefore the wellbeing of your child may be affected. Please ensure you bring your child’s Red Health Record with you to the appointment also.

I will require a completed consent form which I will go through with you to ensure you are able to make the right informed care choice for you and your baby. This will be completed before any assessment or treatment is given and will state that you have read my Privacy Policy.

Information storage

I am fully registered with the Information Commisioners Office and practice in accordance with the Data Protection Act. All records are stored within a Healthcare Software programme Cliniko and are the property of Jas Jones, Registered Midwife, Tongue Tie Practitioner and IBCLC. Please see the Privacy Policy below or ask for further details if required.

Currently, I do not accept any payments by cheque and at Home Visits I am able to accept cash and card payments. In clinic, both cash and card payments are accepted.

Complaints policy
I hope that you will not be unhappy with my service, however if you do feel that there is a need to make a complaint it will be taken very seriously. Please contact me directly in writing to share your concerns and I will endeavour to resolve your complaint as quickly as possible.

As a registrant of the Nursing and Midwifery Council (NMC) as a Registered Midwife, I am required to maintain professional standards in all areas of my practice which includes record keeping and the safe storage of my documents. This is clearly covered in the NMC Code of Professional Standards and Behaviour for Nurses and Midwives.

What information is being collected?

Within the consultation we will discuss family medical history, information regarding your birth and you and your baby’s feeding journey.  
I will request a degree of personal information about you/your baby including date of birth, address, phone numbers, email address, NHS Numbers, GP details and others.
I also have an online feedback form to collate statistics on aspects of my services .

What if parents/ clients notice there are inaccuracies in the documentation?

If you notice there are any inaccuracies in my documentation, please do contact me as soon as possible so that I can amend accordingly and send you the updated records.

Can parents/ clients request a copy of the details stored?

You can request to see any documentation that I have about you and your baby. With your permission, I will be recording the care I provide in your baby’s Personal Child Health Record (red book) and sending you an electronic copy of your baby’s records.

Who is collecting it?

I work as a Private Tongue Tie Practitioner and IBCLC alone, therefore only I will be taking details of your medical history and answering my mobile should you call/text or email.

How is it collected?

I will write in your baby’s Personal Child Health Record or, if not available during the consultation, provide a separate page for you to insert into the book.
I also use an electronic healthcare software programme called Cliniko to record care provided within our appointment and ongoing communications.
If I send you electronic information via email, I will have your email address on my email account.
The same applies to your telephone number if we converse on the phone or via text.

Why is it being collected?

The Nursing and Midwifery Council (NMC) regulate the care I provide in accordance with the NMC Code of Professional Standards and Behaviour For Nurses and Midwives and they state that records are kept for 25 years.
I also require the collection of your data, for ongoing continuity of care to ensure that I can give the best care possible.

How will it be used?

Your information will be used in the ongoing care of your current and future baby’s care in follow up appointments or discussions.
Your information will not be shared with anyone without your knowledge or sold on to any other businesses.
The exception to this will be if I have any safeguarding concerns about the safety of your baby or children. I am then professionally and legally obligated to share relevant information with appropriate agencies without your consent to enhance the safeguarding of all children.
Annually, I collate statistics on the number of visits, type of feeding situations, comparison of age, sex of baby and feeding outcomes. The information will be lifted from your notes but collated anonymously for my audit purposes.

How will it be stored?

Electronic notes are kept within Cliniko (password protected software programme) on my password protected laptop. I do not store any  hard copies of any records. Any information stored on my mobile telephone is protected by a passcode.

Who will it be shared with?

Documentation within the Personal Child Health Record and the electronic copy I provide to you will belong to you for you to share with family or other health professionals that may require the information.

Regarding the confidentiality of our conversations, please do not share direct exerts of your notes on social media sites without first discussing with me.
During the appointment, you will be asked to sign a consent form to give me permission provide the expected level of care for your baby and also to create electronic notes and to email you a copy.

The consent form gives me permission to:-


  • Take personal information about you and your baby and then perform an oral assessment of your baby. Following discussion with yourself, then make a suitable care plan which may/may not include a Frenulotomy.
  • Liaise with health professionals as needed.
  • With your explicit permission, take a picture of your baby for professional use only and may appear on my website or social media pages.
  • With your explicit permission I may use your feedback on my website.


What will be the effect of this on the individuals concerned?

That my documentation will provide transparency of care for you as parents.
